Please review the below terms, including The Pick-Up Club’s liability waiver.

To acknowledge agreement, you will agree to the waiver each time you sign up your child for a game through our online booking platform . We appreciate your help in ensuring a safe and fun environment for everyone.

  • I, the parent/guardian, hereby grant permission to The Pick-Up Club to share my child/children’s name with administrators, game captains and the parents of other players in their game.

  • I, the parent/guardian, hereby grant permission to The Pick-Up Club to photograph and/or videotape my child during games. I understand that these photographs and/or videos may be used for promotional and marketing material, including but not limited to websites, social media, brochures, newsletters, and local media publications.

  • The Pick-Up Club reserves the right to cancel or change games at any time. 

RELEASE OF LIABILITY FOR MINOR PARTICIPANTS

I, the parent or guardian of the named minor, acknowledge, agree and understand that:

  • The named minor is in good health and proper physical condition to participate in The Pick-Up Club sports program.

  • There are certain risks and hazards involved in the named minor participating in The Pick-Up Club that may result in injury or death to the minor or other players including, but not limited to, those hazards associated with playing conditions, equipment, and other participants.

  • I hereby release, discharge and agree not to sue The Pick-Up Club, or its owners, officers, agents, servants, associations, employees, or any person or entity connected with The Pick-Up Club for any claim, damages, costs or cause of action which I have or may in the future have as a result of injuries or damages sustained or incurred by the named minor from whatever cause including but not limited to the negligence, breach of contract or wrongful conduct of the parties hereby released.
